It was a relaxing night for Chin to enjoy movie with her family, but it turned out to be nightmare when she got bitten by a rat in the theatre. She posted her picture of her bandaged toe on her Backbone page and immediately the photo had been shared by at least 700 Backbone users on that particular day. Explain and evaluate claims made by linguistic relativists regarding the relationship between language, thought and culture Essay The relationship between language, culture and thought has been a controversial discussion over decades. Many linguists and thinkers have argued that language lead to large differences in culture and thoughts. Some argue that language controls people’s view and thought of the world, where language embodies worldview, and some argue the otherwise. Language, culture and thought may always refer as together, but any one of them implies the other two. In this essay, I am going to focus on linguistic relativism and I will evaluate claims made by linguistic relativists. Linguistic relativism is a weaker interpretation of linguistic determinism. It is â€Å"a window through which to view the cognitive process, not as an absolute. It’s set forth to be used in looking at a phenomenon differently than one usually would. † (Badhesha, 2001) Linguistic categorize and usage influence thought and certain kinds of non-linguistic behavior. Linguistic relativity hypothesis has always been a controversial and serious topic. In late-eighteenth and early nineteenth century, Boas claimed that there’s no intrinsic relationship between culture and language. (Boas, 1911) Acquainted with Boas, Edward Sapir was impressed with Boas’ statement. Later, he proposed a theory which becomes the most famous attempt in demonstrating relationship between language, culture and thoughts, â€Å"Sapir-Whorf Hypothesis† (Whorfian Hypothesis). The Sapir-Whorf Hypothesis can be broken into two simple concepts: Linguistic Determinism and Linguistic Relativism. Linguistic determinism holds a stronger view. It refers to â€Å"the concept that what is said has only some effect on how concepts are recognized by the mind†, â€Å"A strict view that what is said is directly responsible for what is seen by the mind. † (Badhesha, 2002) This stronger point is supported by Sapir. Sapir: â€Å"language and culture are not intrinsically associated† but â€Å"language and our thought-grooves are inextricably interwoven, [and] are, in a sense, one and the same† (1921: 228, 232) Sapir believes that language and culture are not explicitly related but language, culture and thoughts are interwoven that cannot be unwoven each other. Sapir also expressed his view that language affects how we perceive the world, â€Å"Even comparatively simple acts of perception are very much more at the mercy of the social patterns called words than we might suppose. †¦ We see and hear and otherwise experience very largely as we do because the language habits of our community predispose certain choices of interpretation. † (1929, p. 210) Sapir here explained that language determines our thoughts and culture, it affects our views of the world and culture is a product of language. Whorf: â€Å"The background linguistic system of each language is not merely a reproducing instrument for voicing ideas but rather is itself the shaper of ideas. † (1940, â€Å"Science and Linguistics,† Technology Review 42: 229-31, 247-8) In this quote, Whorf proposed the stronger form where language determines thought, language shapes our ideas. â€Å"The world is presented in a kaleidoscopic flux of impressions which have to be organized largely by the linguistic systems in our minds. † (Whorf, 1940a:231) This is a weaker form where Whorf argue that the world is somehow under the influence of our linguistic systems. Both in stronger and weaker form, Sapir-Whorf Hypothesis argue that our thoughts, ideas, behavior and culture are under the influence of language. This hypothesis is supported with evidence. It reflects the hierarchy and projects the importance of respecting senior in Chinese family. We can see that Chinese family, including families in Hong Kong divides and identifies its members. For Chinese people, they perceive seniors as people to respect. Sapir-Whorf Hypothesis is the idea that the language that person speaks will affect their perception of the world and accordingly their behavior and culture. In stronger form, language determines the way we think and what we are capable to think of where as linguistic relativity; the weaker form indicates that our thoughts and culture are under the influence of language. Although there are more and more scientific and real life examples that support Sapir-Whorf Hypothesis, anthropologists and linguists still argues weather the hypothesis is too strong or not today. Many argue that instead of language determining our thoughts and culture, they are in fact inter-related and none of them should be dominating the other two. The Sapir-Whorf Hypothesis has received criticism over the years, and it summed up to be three main criticisms, the idea of causality, the methods and translations. While Whorf and Sapir claimed that language affects our perception of the world and the formation of our cultures. Both of them didn’t include the evolution of language, what made up our language, the cultural values that may have been included while setting language system. It is possible that cultural values that might have developed language helped the way we perceive the world. Humboldt (late 18th century) also questioned Sapir-Whorf’s hypothesis, â€Å"The spiritual traits and the structure of the language of a people are so intimately blended that, given either of the two, one should be able to derive the other from it†¦ language is their spirit, and their spirit is their language; it is difficult to imagine any two things more identical. † He also questioned the causality of such hypothesis. Whether if language is the spirit of thoughts and culture or the otherwise. He claims that it is uncertain which one should be derives and to be the spirit of the others. Another obvious criticism is the methods Whorf’s methods. Some believed that Whorf deliberately translated Hopi language in a certain way to support his own hypothesis, to emphasis another thinking system. It can be interpreted as linguistic determinism and relativism; it remains arguable and controversial whether this hypothesis is too strong. Much evidence arises even until today supporting the hypothesis. It is inevitable that every hypothesis receives criticism, but I truly believe the reason why Whorfianism is still being studied today is because we can never deny the influence of language on our thoughts and culture. Utilization And Case Management At A Hospital Sierra View District Hospitals (SVDH) utilization review (UR) process originates in the Case Management (CM) department. They are responsible for the case review and obtaining social services for those patients in need of services. This paper will describe how this process works and how SVDH compares to another facilitys UR structure. There will also be a discussion about the weaknesses of the SVDH program. Utilization Management at SVDH The CM department is responsible for the UR process at SVDH. This department, which has 13 employees, is staffed as follows: four Case Managers; two licensed clinical social workers; four social services worker; a Department Analyst; and a Wound Care Specialist. An Administrative Director oversees the department. The CM department follows a Utilization Review Plan (Appendix A) which establishes how the CM department will review cases and address potential discrepancies from established best practice guidelines like McKessons InterQual Criteria (McKesson, 2010). The four Case Managers are to conduct concurrent reviews on all Medicare admissions. They evaluate the patient chart and documentation to ensure the correct medical care status of Inpatient or Observation Services is assigned to the patient. If the patient is assigned the correct status then the Case Manager will flag the case for review in three to four days to ensure the status of the patient has not changed to such a degree that either a higher or lower service level is required. If that is the case, the manager will contact the primary care physician and seek an order changing the service level. If the physician does not agree, and the documentation in the case ceases to support the current level of care, the case manager will forw ard the case to the physician advisor for review. If the physician advisor agrees that a new level of care is called for then he/she will contact the physician to seek further clarification as to why the patient needs continued services at the current level. Depending on this conversation, there are four possible outcomes for the patient status. One, the patient status will remain at the current level of care because the initial physician has informed the physician advisor of extenuating circumstances regarding the case. Two, the physician will agree to the conversion of the patient status to either a higher or lower level of care. Three, the physician will not agree with the physician advisor, and the advisor will refer the case for the UR committee for a panel decision, which can overrule the original physician; or Four, the patient can be discharged from the hospital. While Case Managers are working the UR of a patient, the Social Workers are concerned about any after-care the patient may require. The Case Managers and Social Workers work together to come up with a discharge plan beginning on the first day of admission. The Social Workers make sure the patients have all the contact information they may need once discharged, and the Case Managers work to ensure placement of the patient in an extended recovery care facility if needed. SVDHs UR Program Compared to Other Hospitals SVDHs UR program is comparable to that of Jewish Hospital (St. Louis). Prior to 1985, the Center for Medicare and Medicaid Services (CMS) reimbursed hospitals for services rendered. Due to increasing costs to the Medicare Trust Fund, the Diagnostic Related Group (DRG) system was developed to reimburse hospitals based on what the average cost of a particular surgery. UR programs at both hospitals began to change to meet the new challenges instituted by CMS. Utilization Review became Case Management at SVDH, while Jewish Hospital named their program Case Coordination. The staff members at both facilities became more involved in complete patient care, from admission to post-discharge. SVDH experienced the same growing pains that Jewish Hospital did when Observation Services became a new cost savings service line instituted by CMS. The next challenge arose in 1990, when quality initiatives arose. These initiatives have only increased since that time, with the establishment of Quality Initiative Organizations (QIO). The QIOs review hospital data and report results back to CMS. The goal of these organizations is to locate services that do not meet their (CMS) standards. When found the QIO will deny payment for the services. While Jewish Hospital attempts to complete a 100% review for quality indicators on all cases, either concurrent or retrospective, SVDH chose to focus 100% on Core Measure cases only. While Jewish Hospital might be ensuring that all cases have a minimum standard of quality, SVDH complies with Joint Commission standards by only reviewing the Core Measure cases. Weakness of the Program There is a major weakness in the SVDH Case Management program, and it has been a weakness for some time. SVDH CM is not a 24/7/365 program. The Case Managers work Monday through Friday from eight to five. One Case Manager works from eight until noon on Saturday in the emergency room, but there is no coverage on Sunday. This means that the one Case Manager only reviews the cases that were admitted from Friday at 5:00 PM until 8:00 AM Saturday morning. If she has time she will also pick up any additional cases she can prior to leaving at noon, but there is no guarantee of that happening. The entire CM staff will review any cases not previously reviewed on Monday, if the patient is still in-house, or the case will undergo a retrospective review. This can cause continuing care problems for patients. An example of one of these problems recently occurred when a patient was not reviewed due to no CM personnel on staff on Friday evenings. The patient presented to the Emergency Room on Friday, and was subsequently placed in Observation Services at the hospital, This case was not reviewed by the weekend case manager. On Monday morning, the CM personnel started reviewing the Medicare admissions and saw this patient on the CM list. In reviewing the chart, the patient plainly met Inpatient criteria. The attending physician could not be contacted so the case was referred to the physician advisor, who agreed the patient should have been classified as Inpatient. The status was changed as of Monday morning. CMS guidelines state that a status can only be changed upon the presentation of an order, and at no point can an Observation Services case be converted to an Inpatient status retroactively. The patient needed additional therapeutic services that would be provided in a long term care facil ity, but CMS guidelines state that Medicare will only pay for those services if the patient has had a qualifying Inpatient stay in a hospital for a minimum of three calendar days (Centers for Medicare Medicaid Services, 2009). Since this patients status converted to Inpatient on Monday morning, the time spent in the hospital from Friday until conversion on Monday did not count towards the qualifying stay. The quandary then is to either discharge the patient to the care facility without a qualifying stay, thereby making the patient responsible for the entire cost of the care, or keeping the patient in the hospital for an additional two days in order to meet the qualifying stay requirements. The obvious answer is to keep the patient in order for them to obtain the necessary time to earn the qualifying stay. This, however, places a bed out of circulation that could have been used by another patient, and might incur costs that do not meet reimbursement criteria for the hospital. Had a CM staff member been on duty Friday night, the case could have been caught earlier, and the qualifying stay could have begun on Friday instead of Monday. The major obstacle to hiring more Case Managers is, frankly, the cost of salaries. Any additional overhead costs are scrutinized closely in these times of shrinking reimbursements. Prior to bringing on new staff, in any area, the return on the investment is reviewed for a profitability margin. If a profit margin cannot be realized then the likelihood of incurring that additional expense is small. Other projects that bring a greater profitability margin have taken priority over additional staff in the CM department. Conclusion The CM Department administers SVDHs UR program. The CM Department encompasses several different services, all dedicated to providing total care to the patient while in the hospital and post-discharge. SVDHs program mirrors common practices within the industry. Like Jewish Hospital (St. Louis), CM at SVDH has evolved and changed over the years to meet the new demands placed on it by regulatory agencies. The CM Department constantly works with physicians to ensure patients receive the appropriate level of care based on physician documentation and InterQual guidelines. The major weakness in the CM program at SVDH is that it is not a 24/7/365 department. This flaw sometimes places the continuum of care for the patient at odds with the regulatory guidelines, and SVDH has to forego reimbursement for services in exchange for better overall patient care and patient financial security.

Both works of art magnificently depicts the everyday domestic life. True to both styles, they both realistically depict a woman doing an every day task. Vermeer's is that of a milkmaid oaring milk and Millet's is that of a woman baking bread.However, both were painted in very different ways. Whereas Vermeer's work is a prime example of a Dutch Baroque painting with its realistic way of portraying an everyday task, the use of bold colors, the detail, and the deep contrast between light and dark, Millet's work is a prime example of a French Realist painting where the colors are warmer and much more simple. Seeing her run off with someone else would make other people laugh at him, and he is extremely angry, even to the point of cursing her with the words â€Å"Damn her, lewd minx! President Roosevelt and Wilson were both reformers for the rights and voice of the people. Theodore Roosevelt used his presidency to put regulations of businesses and make sure the government is still higher than corporations. Individual Reformers and the government in the progressive era went through a great deal to bring about change in the united states, with teddy Roosevelt’s trust-busting and political reform, improvement of laborers and goods, and the restrictions of child labor and women rights. Teddy Roosevelt became known as the great trustbuster. He didn’t get this name for no reason. Roosevelt could identify which trusts were good and which were bad. He could determine which trusts helped the people and which ones hurt them. Roosevelt would hunt down the bad trusts and take them down. He would destroy the bad trusts and restrain the good trusts from becoming bad trusts. Roosevelt believed that the government should police the corporations of America. (A). They didn’t understand that a working condition should be clean and not filled with disease. A lot of the workers had tuberculosis while they worked, and it would get in the meat that they work with, which provided for unsafe meat for people to eat. (B) In other businesses it was just the same. Labors were never treated fairly. Up until recently laborers were not even allowed to go on strike. The government was not helping people; they were staying out of business affairs. Up until the age of reform, and the progressive era. The government was then trying to help out laborers and give them rights. Child laborers and women’s rights were also two big things that came up during the progressive era. Children were expected to work alongside grown men. They were subjected to the same harsh conditions as the adults. Children were not able to be educated because they had to work in the factories. A lot of children only went to school for a short time. The educators acted as if the industrial work was good for children. So children had to go through terrible disease ridden conditions of work. C) The children were not helped by the case of Hammer vs. Dagenhart which ruled that child labor is not a government problem; it is to be judged by the state legislatures. Even some parents felt like it was their kids duty to work, and that the children should work. (G) Eventually however the children were set free from the bondage of labor. But women were still subjected to discrimination. Women at this time didn’t have very many rights. Women want to be treated equally and to have a say in government. Women suffrage was big at this time. They felt like they could not do anything with the life that they had. So they pushed for reform, which was not happening at this time.
